POLICIES AND PAPERWORK

For the safety of our clients, patients, and staff, we do not permit unrestrained pets on our property. ALL pets must be on a leash or contained in a pet carrier. The front desk receptionist has extra slip leashes for your convenience. A cardboard pet carrier can also be provided for an additional cost.  If you need a leash or pet carrier, please see our front desk staff before getting your pet out of the car.

For the safety of all animals visiting the practice, we ask that your pet be up-to-date on vaccinations before visiting our rehabilitation and pain management facility.

We require payment at time of service.

We accept cash, check, debit card, Discover, Mastercard, and Visa.
